Psychological adaptation resources of the individual in terms of health
and disease

Abstract. Goal: studying psychological adaptive mechanisms and resources of personality correlated with clinical signs of neuropsychic and somatic disorders. Materials and methods: 680 respondents (50.5% females and 49.5% males) at the age from 20 to 66 have been examined. They were subdivided into 3 nosologic groups: patients with psychosomatic, somatopsychic and mental disorders. The following methods have been used: Integrative Anxiety Test (IAT, 1997); Big Five (BIG-5, 2002); Type of Attitude to Disease (TOBOL, 1987); Life-Purpose Orientatio Тest (LPO, D.A. Leontiev, 1992); the R. Lazarus method for diagnosing stress-coping behaviour. (WCQ, 1988), adapted by L.I. Wasserman, B.V. Iovlev, E.R. Isaeva and others (2009); Method for studying mechanisms of individual’s psychological defence “Life Style Index” (LSI, 1998). Results: Statistically significant differences in the coping behaviour repertoire between healthy persons and patients with somatic, psychosomatic and mental disorders have been determined. It has been revealed that stable patterns of defensive-coping behaviour may affect the development of functional disorders and increase stress vulnerability. Psychological predictors of favourable disease progression and patients’ restoration have been revealed: individual’s strength and activity, emotional stability and readiness to cooperation, leading sense-making motives as well as coping behaviour strategies aimed at social activity, depreciation of difficulties and keeping optimism. Social dissatisfaction in significant areas of individual and environment interaction as well as dominance of psychological defence mechanisms became unfavourable predictive factors and factors that affect the course of a disease. It has been determined that lasting somatic disorder gradually changes stress-coping behaviour repertoire towards its impoverishment, which raises psychological maladaptation risk under disease condition. Conclusions: Psychological adaptation mechanisms play an important role in both development (neglecting) of the disease and formation of attitude toward it as well as in medication adherence. The structure and level of individual’s adaptation mechanisms activity in various patient populations determines different variants of their adjustment to the conditions of the disease. In some cases they correspond to the individual style of psychological adaptation to disease, which has conditioned adaptation resources destruction, while in others they affect the disease course pattern, efficiency of remedial and rehabilitative measures and remission quality.

Relevance

The concept of psychological adaptation has long been engrained in modern psychiatry and medical psychology; it is closely aligned with the basic postulates of biopsychosocial paradigm of mental health [2; 8; 11; 17; 33; 34]. Biopsychosocial model of disease development is a complex multilevel (and multiaxial) representation of a disease, when the interest of medical researchers is focused on both clinical diagnostics, which is based on the establishing and qualifying of biological (biochemical, genetic) and psychic (psychophysiological) dysfunctions and impairments, and studying of patients’ psychological specific features and the character of their social functioning [11; 16; 27; 29]. However, despite a large amount of psychomedical researches in somatic and psychiatric clinical practice, which are focused on complex studying of patients’ psychological qualities, the question of studying adaptation mechanisms (coping with the disease’s stress) and evaluating patients’ personal resources in overcoming the frustrating influence of a disease still remains actual [1; 4; 9; 10; 13; 15; 19; 23; 28; 31]. Personal adaptation potential (PAP) is the adaptation resources of personality, which can serve as a basis for a patient in overcoming the disease and successful recovering [3; 13; 20; 21; 25]; its timely diagnostics will allow doctors and psychologists to predict the patient’s behaviour toward his or her disease and arrange the effective tactics of rehabilitation treatment [14; 24; 26; 27; 30].

At the same time, the questions of proving psychological predictive criteria of the disease course, where personal variables (for example, axiological area), adaptation resources of personality and patient’s stress coping behaviour have in some cases a fundamental importance, are poorly covered by the scientific studies. This determined the relevance of our research.

Goal of research: studying psychological adaptation mechanisms and resources of personality, including personal, value and motivational as well as social and environmental levels of integration, correlated with clinical signs of neuropsychic and somatic disorders.

Materials and methods of research

680 respondents aged from 20 to 66 suffering from various somatic and mental diseases have been examined. They were subdivided into 3 nosological groups: patients with psychosomatic, somatopsychic and mental disorders. The total amount of examinees was 680 persons (50.5% females и 49.5% males), which is represented in Table 1.

Methodical apparatus was selected in accordance with the goal and tasks of the research. Clinical psychological method included several variants of semi-structured interview developed for various groups of test persons, and clinical interview. Clinical characteristics of patients (duration of disease, specifics of its course, effect of treatment and others) were obtained from medical records and interviews with attending doctors. Somatic disorders were evaluated according to the reports of medical specialists. Experimental psychological (empirical) method was represented by a methodical complex of the following techniques: Integrative Anxiety Test (IAT, 1997); Big Five (BIG-5, 2002); Type of Attitude to the Disease (TOBOL, 1987), Life-Purpose Orientations Тest (LPO, D.A. Leontiev, 1992); the P. Lazarus method for diagnosing stress-coping behaviour (WCQ, 1988) adapted by L.I. Wasserman, B.V. Iovleva, E.R. Isaeva (2009); method for studying psychological defence mechanisms of personality "Life Style Index" (LSI, 1998).

Research results

It is well known that "systematic problem solving" and "seeking social support" strategies form a basis for functionally adequate, adaptive coping or active coping behaviour [18; 23] (Lazarus and Folkman, 1984). "Avoidance" is one of the leading coping strategies during the formation of maladaptive behaviour. Its use is determined by underdeveloped personal-environmental coping resources and skills of active problem solving.

We studied the specific features of defensive coping behaviour of patients from different nosological groups and established that coping behaviour of inpatients with some pathology significantly differed from coping behaviour of "relatively healthy" persons (p < 0.05).

Thus, patients with such psychosomatic disorders as rheumatoid arthritis (RA) and hypertensive disease (HD) were characterized by differential peculiarities in the repertoire of defensive coping behaviour (see Table 2). RA patients were characterized by the dominance of problem-focused coping strategies — high self-control and seeking support against the background of high level tension of negation and reaction formations defences. Excessive volitional control over emotions and actions turned out to be the specific feature of coping with stress, which is peculiar for these very patients. This behaviour style, from the viewpoint of personality disposition and its influence on the development of a disease, characterises these patients as persons who endure difficulties stoically, suppress spontaneous activity and experiences, which can lead to insufficient emotional reacting, and, as a consequence, to the transition of long-term psychic tension into somatic level. In particular, unconscious disregard of traumatic, psychologically unacceptable information combined with the behaviour suppressing any spontaneity and excessive control over one’s emotions and actions (especially negative) as well as their constant restraining, in its turn, is reflected in the muscular level and passes into constant muscular tension (defensive "muscular armour"), which gradually leads to constraint, stiffness, deterioration in functioning of these body areas and development of pain senses. Pain syndrome that accompanies progression of the disease, in its turn, causes striving for restricting movements and intensifies emotional tension; thus, so-called "vicious circle" is created.

Patients with hypertensive disease (HD) significantly more often tried to emotionally distance from occurring difficulties and avoid problems (run—avoidance and distancing strategies) and were worse in coping with emotions (self-control) than healthy persons (p < 0.01). Besides, they were characterized by general weakness of all psychological defence mechanisms. HD patients showed significant misbalance of mechanisms of psychological adaptation to stress: high search activity was interspersed with problem avoiding and ability to distance from troubles. This defensive coping style characterises HD patients as active persons who are indeed not successful in coping with emotions and try to avoid problem solving. Our study coincides with the ideas of other authors about psychological type and specific personal features of hypertensive patients [7; 8; 26; 30].

One can assume on this basis that personality or, to be more exact, personal traits or fixed patterns of behaviour and attitudes can influence the the development of various functional disorders, symptoms or even the form of a disease as well. This allows to define the places of least resistance (vulnerability to stress), which in some circumstances act as "trigger mechanism" in the development of a stress-caused disease. Moreover, the given studying of HD and RA patients, carried out from the viewpoint of psychological adaptation mechanisms proves to a greater degree the "personal variant" [5; 34] of psychosomatic pathology development.

Unlike psychosomatic disorders, severe somatic diseases including chronic kidney disease (CKD) are not associated in their origin with unsuccessfulness of adaptation systems of psyche, which led to the development of a disease. On the contrary, it is the disease that represents a stressful situation for the sick person and is often accompanied by vital threat, which requires the highest tension of all the available adaptation resources of personality for the adjustment to life situation, determined by the disease. A group of patients, which represents an utmost example of serious somatic disease that threatens life and dooms to serious lifelong restrictions in social functioning and, in general, human life and activity was selected for comparative analysis. It included CKD patients, receiving hemodialysis treatment.

It turned out that the presence of direct vital threat makes sick persons elaborate specific style of coping with stress (see Table 3). Patients tried to solve the occurring problems actively and timely and were prone to higher self-control. They were cautious and anxious to things around them. It is worthy of note that CKD patients seldom used "positive revaluation" strategy.

Thus, the existence of threat or proximity to life-threatening situation determines coping behaviour in a large degree, i.e.: 1) vulnerability to the sources of threat increases, the events are not depreciated, on the contrary, the proneness to overestimate the importance of coming signals is observed; 2) there is neither avoidance nor proneness to run from the occurring stressful events; 3) frequent use of "self-control" strategy. At the same time, patients try to distance from life problems in order to keep forces for coping with the disease as a main stressor. The awareness of the fact of life threatening disease and inability to find a way-out from this situation determines patients’ higher need for support and compassion; however no striving for seeking support in social environment is observed. The obtained data are aligned with the results of other researchers [6; 9; 32].

The group of patients with various maxillofacial pathology (MFP), who suddenly became sick and referred to the hospital after traumas, crashes or unsuccessful medical manipulations, was taken in the study as another pole of somatic diseases. Their disorders were characterised by short period of disease, acute course with expressed pain syndrome, lack of threat to life and, to some degree, a favourable outcome. The research showed that patients with MFP turned out to be less emotionally restrained; besides, they had less control over their feelings. On the one hand, patients worried about their state, as in most cases it was accompanied by pain. On the other hand, patients were likely to underestimate the severity of an illness, because their disease was characterised by sudden development, and regarded their state as temporary.

The study has shown that acutely occurred somatic disease provokes the decrease of self-control. At the initial stages of a disease, the unconscious adaptation mechanisms as urgent methods of reaction to stress are strengthened, the whole spectrum of conscious coping strategies is used actively, aggressive efforts to change the situation are made; besides, there is a tendency to avoid and distance from unpleasant information.

As the disease progresses, the tension of the majority of psychological defence mechanisms reduces. The awareness of a disease is gradually taking place due to the weakening effect of defences, and the ability to develop an adequate inner pattern of a disease increases.

Thus, cognitive revaluation, or re-thinking of a situation, acquired greater importance for patients with lingering, chronic inflammatory processes. It is connected with inability to influence on the course of treatment, experiences of uncertainty of their state and further prognosis, gradual alteration of consciousness and deeper understanding (awareness) of the processes that happen to them.

Studying the specific features of psychological adaptation of HIV infected patients caused a special scientific interest. The fact of discovering incurable, progressive, life-threatening disease cannot but affect psychological adaptation mechanisms. Attitude of the society to this disease plays an important role, which also affects patients’ emotional state and behaviour [12]. The study showed that the repertoire of defensive coping behaviour of this group of patients differed from other groups of patients and the majority of healthy people to a great degree (p < 0.001), making it clear that those people had encountered the most severe life reality (significant differences are obtained according to virtually all indexes). HIV patients were emotionally instable, aggressive and conflictive (confrontation), tended much more often to avoidance and emotional escape from problems (distancing); they were more passive in overcoming difficulties and preferred more often to ask for aid and support people around them (seeking social support). It should be noted that premorbid characteristics and adaptation resources of patients’ personality determined to a large extent the specifics of coping style in the conditions of the developed disease.

Patients with endogenic mental disorders were regarded in our study as the example of extremely expressed psychic maladaptation and disintegration of personality. Studying of coping behaviour repertoire has shown that patients with mental disorders tend to rely on social support in a greater degree than the rest of the groups; it confirms the importance of social supportive network for their general state and recovery (see Table 4).

The expressed tendency to avoid problems is specific for the patients: they try to hide from problems and find moral and emotional support in stressful situations. The difficulties of the development of active problem-solving behaviour in mentally ill persons was determined by cognitive violations caused by the disease and by current mental state: low ability to adequate appraisal of the events and, respectively, insufficient constructive activity. This is coherent with the opinion of other researchers, who point out that "inadequate appraisal of a situation leads to inadequate emotions, which, in their turn, violate the adaptation responses of a body" (Kaiser S., Scherer K.R., 1998) [cit. ex: 1, p. 46]. Some coping strategies that appear to be constructive coping methods for mentally healthy persons turned out to be maladaptive for patients with schizophrenia. The strategy focused on isolation and escape from social contacts turned out to be extremely unfavourable method of coping with stress that negatively affects the patients’ functioning. In general, results of our research of coping behavior of patients with schizophrenia have a lot in common with results of foreign researches [15; 19; 22; 28].

The studying of adaptation resources in various groups of patients allowed to reveal a number of factors that affect the course of a disease and quality of remission.

It was proved statistically that readiness for cooperation as a personal characteristic is prognostically desirable sign (condition) of a favourable course of a disease. Emotional stability contributed to the fulfillment of recommendations and performing of prophylactic programs (p < 0.04).

Axiological sphere of personality was the main personal resource for patients’ successful recovery. We consider that sense-making motives of a patient’s personality determine the style of defensive coping behaviour in the conditions of chronic somatic disease and are responsible for its course to a large extent. Thus, the research showed that postoperative period was more favourable in patients characterised by high indexes of activity, intensity of life process as well as meaningfulness and interest to life (p < 0.05).

Trait anxiety allowed patients to timely identify the first signs of their disease and follow regime of treatment and medical recommendations thereby defining the quality and effectiveness of treatment. However, hypersensitivity and low tolerance to stress can, in its turn, determine instability of psychic homeostasis and rapid development of psychic maladaptation signs.

The force of "Ego" and activity of personality played a great role in the development of successful types of defensive coping behaviour.

Besides, the study has shown that low satisfaction of an individual’s relationships in different areas of life, lack of interests in the outer circle of life and low intensity of life process contributed to slow recovery, persisting unwellness of patients in the postoperative period (p < 0.05) and their fixation on their health.

Discussion

The study has revealed the psychological predictors of the favourable course of the disease and recovery of patients with somatic and mental disorders. Such personal variables as force and activity of personality, emotional stability and readiness for cooperation as well as leading sense-making motives (interest, meaningfulness and intensity of life process) form a basis for personal adaptation potential in the conditions of disease.

The studying of coping and defensive behaviour in the conditions of a disease has shown that mechanisms of psychological adaptation play an important role in both development (triggering) of the disease and formation of disease behaviour; they affect the character of disease experiences and, as a consequence, its course.

Coping strategies, aimed at social activity, devaluating of difficulties and keeping optimism had a favourable predictive meaning. Social dissatisfaction in the significant areas of individual and environment interaction as well as dominance of psychological defence mechanisms (regression, substitution and reaction formations) over problem-solving strategies of coping behaviour became unfavourable predictive factors affecting the course of a disease. The expressiveness of negation and reaction formations defences in the structure of personality together with run—avoidance, confrontation and distancing coping strategies led to the chronization of emotional tension and aggravation of symptoms and supressed the development of adequate pattern of a disease, awareness of one’s role in deterioration of general state and creation of adequate compliance. Such patients neglected the signals of any somatic or psychological ill-being that came from the body. Ignoring problems, striving to supress emotions and avoid problems together with direct rigid strategies of coping with stress represent a stable defensive coping style of behaviour, which enhances the severity of a state and quickens the progress of a disease.

In its turn, the disease itself can dramatically change an individual’s lifestyle and sense of meaning and purpose in life. Habitual and earlier learned strategies of coping with difficulties, which turned out to be useless in the conditions of a severe disease, change under the influence of the disease. The study established the specific features of psychological adaptation to a disease: lingering and severe somatic disorder gradually changes the repertoire of stress coping behaviour toward its impoverishment, which is associated with the risk of development of impaired psychological adaptation under the condition of a disease. According to our data, as the duration and severity of a disease increases the general tendency to submission is observed and repression defensive mechanism strengthens. Revealed differences in coping behaviour depend on the degree of danger to life posed by the disease. Dependence on medical institution and inability to recover form a passive position of an individual; patients prefer to emotionally estrange themselves from the experiences which are not connected with health.

In conclusion it can be assumed that the structure and level of activity of personality adaptation mechanisms in various types of patients determine various options of their adjustment to the conditions of the disease. In some cases they correspond to the individual style of psychological adaptation previous to the disease, which has determined the destruction of adaptation resources, while in others they affect the disease course pattern, efficiency of remedial and rehabilitative measures and remission quality.

Аннотация. Актуальность. В современных исследованиях, направленных на комплексное изучение медико-биологических и социально-психологических характеристик больных, по-прежнему актуальной является проблема изучения адаптационных механизмов и личностных ресурсов больных по преодолению болезни. Своевременная их диагностика позволит врачам и психологам прогнозировать поведение больного в отношении своего заболевания и выстроить эффективные тактики восстановительного лечения. Цель: изучение психологических адаптационных механизмов и ресурсов личности в соотношении с клиническими проявлениями нервно-психических и соматических расстройств. Материал и методы: обследовано 680 респондентов (50,5% женщин и 49,5% мужчин) в возрасте от 20 до 66 лет, составивших 3 нозологические группы: пациенты с психосоматическими, соматопсихическими и психическими расстройствами. Использовались методики: «Интегративный тест тревожности» (ИТТ, 1997); «Большая пятерка» (BIG-5, 2002); «Тип отношения к болезни» (ТОБОЛ, 1987); «Тест смысложизненных ориентаций» (СЖО, Леонтьев Д.А., 1992); Методика для диагностики совладающего со стрессом поведения Р. Лазаруса (WCQ, 1988) в адапт. Вассермана Л.И., Иовлева Б.В., Исаевой Е.Р. и др. (2009); Методика для изучения механизмов психологической защиты личности «Индекс жизненного стиля» (ИЖС, 1998). Результаты: установлены статистически значимые различия в репертуаре совладающего поведения между здоровыми лицами и пациентами с соматическими, психосоматическими и психическими расстройствами. Обнаружено, что устойчивые паттерны защитно-совладающего поведения могут влиять на формирование функциональных расстройств и повышать уязвимость стрессу. Выявлены психологические предикторы благоприятного течения болезни и восстановления больных: сила и активность личности, эмоциональная стабильность и готовность к сотрудничеству, ведущие смыслообразующие мотивы, а в копинг-поведении — стратегии, направленные на социальную активность, обесценивание трудностей, сохранение оптимизма. Прогностическими неблагоприятными факторами, оказывающими влияние на течение заболевания, являлись: социальная неудовлетворенность в значимых сферах личностно-средового взаимодействия, преобладание механизмов психологической защиты. Установлено, что длительное соматическое расстройство постепенно изменяет репертуар совладающего со стрессом поведения в сторону его обеднения, что повышает риск развития нарушений психической адаптации уже в условиях болезни. Выводы: механизмы психологической адаптации играют важную роль как в развитии («запускании») болезни, так и в формировании отношения к ней, приверженности к лечению. Структура и уровень активности адаптационных механизмов личности у разных контингентов пациентов определяют различные варианты их приспособления к условиям болезни. В одних случаях они корреспондируют с индивидуальным стилем психологической адаптации до болезни, обусловившим слом адаптационных ресурсов, в других — влияют на характер течения болезни, эффективность лечебных, восстановительных мероприятий и качество ремиссии.

Актуальность темы

В современной психиатрии и медицинской психологии уже достаточно давно и прочно укоренилась концепция психической адаптации, которая тесно перекликается с основными постулатами биопсихосоциальной парадигмы психического здоровья [2; 8; 11; 17; 33; 34]. Биопсихосоциальная модель развития заболевания — комплексное многоуровневое (и многоосевое) представление о заболевании, когда помимо клинической диагностики, производимой на основе установления и квалификации биологических (биохимических, генетических) и психических (психофизиологических) дисфункций и нарушений, интерес исследователей-медиков направлен также на изучение психологических особенностей пациентов, на характер их социального функционирования [11; 16; 27; 29]. Однако, несмотря большое количество медико-психологических исследований в соматической и психиатрической клинике, направленных на комплексное изучение психологических характеристик больных, по-прежнему актуальной остается проблема изучения адаптационных механизмов (совладания со стрессом болезни) и оценки личностных ресурсов больных по преодолению фрустрирующего влияния болезни [1; 4; 9; 10; 13; 15; 19; 23; 28; 31]. Личностный адаптационный потенциал (ЛАП) — это адаптационные ресурсы личности, которые могут послужить опорой для больного в преодолении болезни и успешном восстановлении [3; 13; 20; 21; 25], а его своевременная диагностика позволит врачам и психологам прогнозировать поведение больного в отношении своего заболевания, выстроить эффективные тактики восстановительного лечения [14; 24; 26; 27; 30].

Вместе с тем, в научных исследованиях недостаточно освещены вопросы обоснования психологических прогностических критериев течения болезни, где личностные переменные (например, ценностно-смысловая сфера), адаптационные ресурсы личности и совладающее со стрессом поведение больного имеют подчас фундаментальное значение. Это и определило актуальность нашего исследования.

Цель исследования: изучение психологических адаптационных механизмов и ресурсов личности, включая личностный, ценностно-мотивационный и социально-средовой уровни интеграции в соотношении с клиническими проявлениями нервно-психических и соматических расстройств.

Материал и методы исследования

Были обследованы больные в возрасте от 20 до 66 лет, страдающие различными соматическими и психическими заболеваниями, составившие 3 нозологические группы — больные с психосоматическими, соматопсихическими и психическими расстройствами. Общее количество обследованных составило 680 человек (50,5% женщин и 49,5% мужчин) и представлено в таблице № 1.

Методический аппарат был подобран в соответствии с целью и задачами исследования. Клинико-психологический включал несколько вариантов полуструктурированного интервью, разработанных для разных групп испытуемых, и клиническую беседу. Клинические характеристики больных (длительность болезни, особенности течения, эффект лечения и другие) были получены из медицинской документации и бесед с лечащими врачами. Оценка соматических расстройств проводилась по заключениям врачей-специалистов. Экспериментально-психологический (эмпирический) метод был представлен методическим комплексом из следующих методик: «Интегративный тест тревожности» (ИТТ, 1997); «Большая пятерка» (BIG-5, 2002); «Тип отношения к болезни» (ТОБОЛ, 1987); «Тест смысложизненных ориентаций» (СЖО, Леонтьев Д.А., 1992); Методика для диагностики совладающего со стрессом поведения Р. Лазаруса (WCQ, 1988) в адаптации Вассермана Л.И., Иовлева Б.В., Исаевой Е.Р. и др.(2009); Методика для изучения механизмов психологической защиты личности «Индекс жизненного стиля» (ИЖС, 1998).

Результаты исследования

Как известно, основу функционально-адекватного, адаптивного совладания или активного копинг-поведения составляют стратегии «планомерное разрешение проблем» и «поиск социальной поддержки» [18; 23] (Lazarus and Folkman, 1984; 1988). При формировании дезадаптивного поведения одной из ведущих является копинг-стратегия «избегание». Ее использование обусловлено недостаточностью развития личностно-средовых копинг-ресурсов и навыков активного разрешения проблем.

Нами были изучены особенности защитно-совладающего поведения пациентов разных нозологических групп, и было установлено, что копинг-поведение больных людей, находящихся на стационарном лечении с той или иной патологией, достоверно отличалось от копинг-поведения «условно здоровых» лиц (p<0,05).

Так, например, больные с психосоматическими расстройствами, такими, как ревматоидный артрит (РА) и гипертоническая болезнь (ГБ), характеризовались отличительными особенностями в репертуаре защитно-совладающего поведения (см. табл. № 2). У больных РА на фоне высокого уровня напряженности защит отрицание и реактивные образования доминировали проблемно-фокусированные копинг-стратегии — высокий самоконтроль и обращение за поддержкой. Особенностью совладания со стрессом, характерной именно для этих больных, оказался чрезмерный волевой контроль над эмоциями и действиями. Данный стиль поведения, с точки зрения личностной диспозиции и ее влияния на развитие заболевания, характеризует этих пациентов как стоически переносящих трудности людей, сдерживающих спонтанную активность, подавляющих переживания, что может приводить к недостаточному эмоциональному отреагированию, и, как следствие, переходу длительного психического напряжения на соматический уровень. В частности, бессознательное игнорирование травмирующей, психологически неприемлемой информации в сочетании с поведением, подавляющим любую спонтанность, чрезмерное контролирование своих эмоций и действий (особенно негативных), постоянное их сдерживание, в свою очередь, отражается на мышечном уровне, переходя в постоянное мышечное напряжение (защитный «мышечный панцирь»), постепенно приводит к скованности, зажатости, ухудшению функционирования данных участков тела и формированию болевых ощущений. Болевой синдром, сопутствующий прогрессированию заболевания, в свою очередь, вызывает стремление к ограничению движений, усиливает эмоциональное напряжение, создается так называемый «порочный круг».

Больные гипертонической болезнью (ГБ) достоверно чаще стремились эмоционально дистанцироваться от возникающих трудностей и избегать проблем (стратегии бегство—избегание и дистанцирование), хуже справлялись с эмоциями (самоконтроль), чем здоровые люди (p<0,01). Кроме того, они отличались общей слабостью всех механизмов психологической защиты. У больных ГБ наблюдался выраженный дисбаланс механизмов психологической адаптации к стрессу: высокая поисковая активность чередовались с избеганием проблем и умением дистанцироваться от неприятностей. Данный защитно-совладающий стиль характеризует больных ГБ как людей активных, но которые плохо справляются с эмоциями и стремятся к избеганию решения проблем. Наше исследование согласуется с представлениями других авторов о психологическом типе и личностных особенностях гипертоника [7; 8; 26; 30].

Исходя из этого, можно утверждать, что личность, а точнее личностные черты или устойчивые паттерны поведения и установки, могут также влиять на формирование тех или иных функциональных расстройств, симптомов или даже формы заболевания. Это позволяет определить места наименьшего сопротивления (уязвимости стрессу), являющиеся при определенных обстоятельствах «пусковым механизмом» в развитии связанного со стрессом заболевания. Более того, представленное исследование больных ГБ и РА, проведенное через призму механизмов психологической адаптации, доказывает в большей степени «личностный вариант» [5; 34] развития психосоматической патологии.

В отличие от психосоматических расстройств, тяжелые соматические заболевания, как например, хроническая почечная недостаточность (ХПН), в своем происхождении не связаны с неуспешностью адаптационных систем психики, приведших к развитию заболевания. Наоборот, сама болезнь представляют стрессовую ситуацию для болеющего человека, нередко сопровождающуюся витальной угрозой, что требует наивысшего напряжения всех имеющихся адаптационных ресурсов личности для приспособления к жизненной ситуации, определяемой болезнью. Для сравнительного исследования была отобрана группа пациентов, являющаяся крайним примером тяжелого соматического, угрожающего жизни и обрекающего на пожизненные серьезные ограничения в социальном функционировании и, в целом, жизнедеятельности человека, заболевания — это пациенты с ХПН, находившиеся на лечении гемодиализом.

Оказалось, что наличие непосредственной витальной угрозы заставляет больных людей вырабатывать определенный стиль совладания со стрессами (см. табл. № 3). Пациенты стремились активно и своевременно решать возникающие проблемы, были склонны к повышенному самоконтролю, настороженно и опасливо относиться к происходящему. Примечательно, что пациенты с ХПН редко использовали стратегию позитивная переоценка.

Таким образом, факт существования угрозы или близость к ситуациям, угрожающим жизни, в значительной степени определяет копинг-поведение, а именно: 1) повышается восприимчивость к источникам угрозы, события не обесцениваются, наоборот, наблюдается склонность к гипероценке важности поступающих сигналов; 2) отсутствует избегание, нет склонности к бегству от возникающих стрессовых событий; 3) частое использование стратегии «самоконтроль». Вместе с тем, больные стараются дистанцироваться от житейских проблем с целью сохранения сил для совладания с болезнью как самым главным стрессором. Осознание факта угрожающего жизни заболевания и невозможность выхода из создавшегося положения определяет повышенную потребность больных в поддержке и сочувствии, однако не наблюдается стремления к поиску помощи в социальном окружении. Полученные данные перекликаются с результатами других исследователей [6; 9; 32].

В качестве другого полюса соматических заболеваний в исследование была взята группа пациентов с различного рода челюстно-лицевой патологией (ЧЛП), которые внезапно заболели и обратились в стационар после травм, аварий или неудачных медицинских манипуляций. Их расстройства характеризовались малым сроком заболевания, острым течением с выраженным болевым синдромом, отсутствием угрозы для жизни и в какой-то степени благополучным исходом. По данным исследования, больные с ЧЛП оказались наименее эмоционально сдержанными и хуже умели контролировать свои чувства. С одной стороны, пациенты были озабочены своим состоянием, поскольку в большинстве своем оно сопровождалось болью. С другой стороны, пациенты склоны были недооценивать серьезность болезни, поскольку их заболевание характеризовалось внезапным развитием, относились к своему состоянию, как к временному.

Исследование показало, что остро возникшее соматическое заболевание провоцирует снижение самоконтроля. На начальных этапах заболевания усиливаются бессознательные механизмы адаптации в качестве «пожарных» способов реагирования на стресс, активно используется весь спектр сознательных стратегий совладания, прилагаются агрессивные усилия по изменению ситуации, а также тенденция к избеганию и дистанцированию от неприятной информации.

По мере прогрессирования болезни снижается напряженность большинства механизмов психологической защиты. За счет ослабления действия защит постепенно происходит осознание болезни и повышается способность к формированию адекватной внутренней картины болезни.

Так, например, для пациентов с затяжными, хроническими воспалительными процессами бóльшее значение приобретала когнитивная переоценка, переосмысление ситуации. Это связано с невозможностью повлиять на ход лечения, переживаниями неопределенности своего состояния и дальнейшего прогноза, с постепенной перестройкой сознания и более глубоким пониманием (осознанием) происходящих с ними процессов.

Особый научный интерес вызвало изучение особенностей психологической адаптации у ВИЧ-инфицированных пациентов. Факт обнаружения неизлечимого, прогрессирующего, угрожающего жизни заболевания не может не сказываться на механизмах психологической адаптации. Немаловажную роль играет отношение общества к данному заболеванию, что также влияет на эмоциональное состояние и поведение больных [12]. Исследование показало, что репертуар защитно-совладающего поведения данной группы пациентов в значительной степени (p<0,001) отличался от других групп больных и от большинства здоровых людей, явно указывая на то, что эти люди столкнулись с особенно тяжелой жизненной реальностью (достоверные различия получены практически по всем показателям). Больные с ВИЧ были эмоционально неуравновешенны, агрессивны и конфликтны (конфронтация), значительно чаще стремились к избеганию и эмоциональному отстранению от проблем (дистанцирование), более пассивны в преодолении трудностей и чаще предпочитали обращаться за помощью и поддержкой к окружающим (поиск социальной поддержки). Следует отметить, что преморбидные особенности и адаптационные ресурсы личности пациентов во многом определяли специфику стиля совладания в условиях уже развившегося заболевания.

Пациенты с эндогенными психическими расстройствами в нашем исследовании рассматривались как пример крайней степени выраженности психической дезадаптации и дезинтеграции личности. Изучение репертуара копинг-поведения показало, что больные с психическими расстройствами склонны в большей степени, чем все остальные группы больных, опираться на социальную поддержку, это подтверждает важность для их самочувствия и восстановления наличия социальной поддерживающей сети (см. табл. № 4).

Характерна выраженная тенденция к избеганию проблем: в стрессовых ситуациях они стараются спрятаться от проблем, найти моральную и эмоциональную поддержку. Формирование активного проблемно-решающего поведения у психически больных было затруднено из-за когнитивных нарушений, вызванных болезнью, и актуального психического состояния: наблюдалась низкая способность к адекватной оценке происходящих событий и, соответственно, недостаточная конструктивная активность. Это согласуется с мнением других исследователей, подчеркивавших, что «неадекватная оценка ситуации приводит к неадекватным эмоциям, которые, в свою очередь, нарушают адаптивные реакции организма» (Kaiser S., Scherer K.R., 1998) [Цит. по: 1, с. 46]. Некоторые копинг-стратегии, являющиеся конструктивными способами совладания для психически здоровых людей, оказывались дезадаптивными для больных шизофренией. Крайне неблагоприятным способом совладания со стрессом, негативно отражающимся на функционировании пациентов, оказалась стратегия, направленная на изоляцию и уход от социальных контактов. В целом, результаты нашего исследования копинг-поведения у пациентов шизофренического спектра во многом перекликаются с результатами зарубежных исследований [15; 19; 22; 28].

Исследование адаптационных личностных ресурсов у разных групп больных позволило выявить ряд факторов, влияющих на течение заболевания и качество ремиссии.

Статистически было доказано, что готовность к сотрудничеству как личностная характеристика является прогностически желательным признаком (условием) благоприятного течения болезни. Эмоциональная стабильность способствовала соблюдению рекомендаций и выполнению профилактических программ (p<0,04).

Ценностно-смысловая сфера личности являлась главным личностным ресурсом для успешного восстановления больных. Мы считаем, что смыслообразующие мотивы личности больного определяют стиль защитно-совладающего поведения в условиях хронической соматической болезни и во многом обусловливают ее течение. Так, в результате проведенного исследования установлено, что у пациентов, характеризующихся высокими показателями активности, насыщенности жизненного процесса, осмысленности и интереса к жизни послеоперационный период протекал более благоприятно (p<0,05).

Тревожность, как личностная черта, способствовала своевременному выявлению пациентами первых признаков своего заболевания, соблюдению лечебного режима и врачебных рекомендаций, тем самым определяя качество и эффективность лечения. Однако повышенная чувствительность, низкая толерантность к стрессу может обусловливать, в свою очередь, неустойчивость психического гомеостаза и быстрое развитие признаков психической дезадаптации.

Для формирования успешных типов защитно-совладающего поведения важное значение имели сила «Я» и активность личности.

Исследование также показало, что низкая удовлетворенность отношениями личности в разных сферах жизни, отсутствие интересов во внешнем круге жизни, низкая степень насыщенности жизненного процесса способствовали медленному восстановлению, сохраняющемуся плохому самочувствию больных в послеоперационный период (p<0,05), фиксации пациентов на собственном здоровье.

Обсуждение результатов

В исследовании выявлены психологические предикторы благоприятного течения болезни и восстановления больных соматическими и психическими расстройствами. Такие личностные переменные, как сила и активность личности, эмоциональная стабильность и готовность к сотрудничеству, ведущие смыслообразующие мотивы (интерес, осмысленность и насыщенность жизненного процесса), составляют основу личностного адаптационного потенциала в условиях болезни.

Исследование совладающего и защитного поведения в условиях болезни показало, что механизмы психологической адаптации играют важную роль как в развитии («запускании») болезни, так и в формировании поведения в отношении к болезни, они влияют на характер переживания болезни и, как следствие, на ее течение.

Благоприятное прогностическое значение имели копинг-стратегии, направленные на социальную активность, обесценивание трудностей, сохранение оптимизма. Прогностическими неблагоприятными факторами, оказывающими влияние на течение заболевания, являлись: социальная неудовлетворенность в значимых сферах личностно-средового взаимодействия, преобладание механизмов психологической защиты (регрессия, замещение и реактивные образования) над проблемно-решающими стратегиями копинг-поведения. Выраженность в структуре личности защит отрицание и реактивные образования в сочетании с копинг-стратегиями бегство—избегание, конфронтация и дистанцирование вели к хронизации эмоционального напряжения и усилению симптоматики, препятствовали формированию адекватной картины болезни, осознанию своей роли в ухудшении самочувствия, созданию полноценного комплайенса. Такие пациенты невнимательно относились к сигналам какого-либо соматического и психологического неблагополучия, поступающим из организма. Игнорирование проблем, стремление сдерживать эмоции и уклоняться от проблем в сочетании с прямолинейными негибкими стратегиями совладания со стрессом — это устойчивый защитно-совладающий стиль поведения, который лишь усугубляет тяжесть состояния и ускоряет прогрессирование заболевания.

В свою очередь, сама болезнь может изменить коренным образом весь стиль жизни и смысловые ориентиры личности. Под влиянием болезни изменяются привычные и выученные ранее, но оказавшиеся непригодными в условиях тяжелого заболевания стратегии преодоления трудностей. В исследовании установлены особенности психологической адаптации к болезни: длительное и тяжелое соматическое расстройство постепенно изменяет репертуар совладающего со стрессом поведения в сторону его обеднения, что сопряжено с риском развития нарушений психической адаптации уже в условиях болезни. По нашим данным, с увеличением длительности и тяжести болезни наблюдается общая тенденция к смирению, усиливается защитный механизм вытеснение. Различия в копинг-поведении обнаруживаются в зависимости от того, насколько опасным для жизни является заболевание. Зависимость от лечебного учреждения, невозможность выздоровления формируют пассивную позицию личности, больные предпочитают эмоционально отстраняться от переживаний, не связанных со здоровьем.

В заключение можно утверждать, что структура и уровень активности адаптационных механизмов личности у разных контингентов пациентов определяют различные варианты их приспособления к условиям болезни. В одних случаях они корреспондируют с индивидуальным стилем психологической адаптации до болезни, обусловившим слом адаптационных ресурсов и развитие заболевания, в других — влияют на характер течения болезни, эффективность лечебных, восстановительных мероприятий и качество ремиссии.
